Feeback
(also known as positive criticism) is how an organization grows,
matures, improves and remains a vital part of any community in the
future, which is the goal of the JFAC (we hope it is yours' as well).
The following survey questions are prepared specifically to glean such
positive feedback. It is completely anonymous (an optional "blank field"
to enter an email address is provided for those who may wish further
communication), so please be open, honest, and free to express your
opinions, suggestions, or concerns (in a positive and constructive
manner). There are no "required" fields, so fill in any or all
appropriate information.
All information will be collected on a regular
basis. Please don't hesitate to forward this on to those you know
